Inactive Status Request

This form is for students who are requesting Inactive Status from either the D.Min program or the D.EdMin program. Once the form is completed the Professional Doctoral Oversight Committee will review the Inactive Request for approval. The student will be notified via email of the Professional Doctoral Oversight Committee's decision.
