Mullen High School wishes to provide the very best coaches it can. The athletic department at Mullen High School believes that athletics and participation in our athletic program is an educational activity that contributes to the physical, personal, social, and spiritual growth of the student athletes involved, and therefore, develops the student-athletes as Lasallian and Christ-like. Coaches play an integral role in this mission. The evaluation will help the athletic department pursue and improve coaches in alignment with these objectives.

To that end, please evaluate your coach honestly. Please evaluate only one coach per form. Please use additional forms as needed to evaluate more than one coach (i.e. assistant coaches, other sports, etc.). This evaluation is anonymous to the coaches and the Athletic Office personnel. Please use the following scale from 1 to 5, where 1 is Strongly Disagree and 5 is Strongly Agree.
