2016 Individual Registration Form
This form should be filled out by the decathlete, coach or chaperone for all students who want to participate in the 2016 USAD Nationals as an individual, whether in Alaska or online. The USAD Nationals Online Essay/Art tests will be held April 7, 2016 at 12:30 p.m. Central Daylight Time. Usernames and passwords are automatically sent to the email address used for registration on usadic.org once payment for the final round is confirmed. Instructions are sent to this address the week of the competition. Practice will take place from April 1 at 12:30 p.m. to April 5 at 12:30 p.m. Central Daylight Time. (International teams will adjust for the time difference to complete the tests.)
