The Student Wellness Center (SWC) is your home for counseling, medical, and well-being needs at SJSU!

Complete this form to submit your request for tabling representation, an information session, or an interview about the Student Wellness Center. Tabling and information sessions will cover all Student Wellness Center resources and services.

Please allow at least 4-6 weeks advance notice to fulfill a request. Following the online submission, requesters may expect to receive a submission confirmation within 3-5 business days. Though we will attempt our very best to match requests, we may not be able to fulfill all requests. Complete the following information so that we may process your request.
