Course Withdrawal Notification

All scholarship recipients are required to complete this form to notify the Caskey Center for Church Excellence that you are withdrawing from a course for the semester. This does not substitute the requirement to complete the withdrawal process with the Registrar Offices of NOBTS or Leavell College. Both processes must be completed.

You are responsible for any funds not returned to the Caskey Center for Church Excellence as well as the per course withdrawal fee. It is also important to remember that all Caskey Scholarship recipients are required to be enrolled in at least one course and the evangelism practicum each semester.
