CONSENT FORM (VID USE)
I would like to include your fan vid, ___________________, as a part of my PhD dissertation, which will be reviewed by members of the Faculty of Arts with the possibility that screen captures from the vid will be used in a book or journal article, or exhibited at a conference presentation.
As detailed in the Participant Information Sheet, different countries have different laws relating to copyright, and if you are identified by name you may put yourself at risk of legal action. In addition, if you have included credits or a water-mark of your creator or studio name in the video, it may be possible to trace your name back to your personal website or blog. If this is the case, you have several options:
- You can be identified as the creator of the video with the knowledge that it may lead any potential readers to your personal website and/or expose you to legal action regarding copyright infringement.
- I can blur or remove your name from the vid itself to protect your identity but still include it in my final thesis and/or any publications or conference presentations.
- You may refuse to allow me to include your vid in my final thesis and/or any publications or conference presentations.
Other than removing credits or blurring watermarks if you expressly request, your vid will not edited, altered, or reposted in any way without your written permission.
By signing below I am indicating my consent to allow a fan vid that I have created to be included in the project entitled “Veni, Vidi, Vids”: Fanvids as Transformative New Media as it has been described to me in the Information Sheet provided by Katie Freund. I agree that my questions about this project have been answered clearly and fully. I have been advised of the potential risks and burdens associated with this project as detailed in the Participant Information Sheet. I understand that my vid may be used for publishing a thesis, book and/or journal articles or used in a conference presentation and I consent for it to be used in that manner.
