Although you may not have been hired yet, this “Request for Work Permit”
Document is only an application for a work permit. If you are under 18 years of age and have not yet graduated from high school you are required to obtain a “Work Permit”. If you have more than one job, you must fill out an application for each job.
- Please fill out the attached application. All information must be filled in, including social security number and birthday. Please do not return this form to us until you have signatures from your supervisor and your parents. All signatures must be original - We cannot accept e-mailed, faxed or copied applications.
- Bring the completed application back to the VCS library. One school day (24 hours) is required to process the permit after dropping off the application. Please return it in person or by US Mail. Please remember your lack of planning does not constitute an emergency on our part.
- Applicant must pick up in person to sign the permit.
