Parent/Guardian Contract
Parent(s) / Guardian(s) Responsibility:
The support of the parents is essential to the success of any youth program. This support, however, should be of the field variety in such activities as fundraising, volunteer hours, equipment maintenance and team sponsorship.
The Cordova Jr. Lancers organization is comprised entirely of volunteers. Coaches, administrators and other volunteers spend time in the off-season, as well as during the season, to provide a football/cheer program for children. Overzealous but well-meaning parents can cause problems if they are not familiar with the policies and philosophies of youth football/cheer. Consequently, the Cordova Jr. Lancers organization has rules concerning the conduct of parents. Please initial each point:
1. Good citizenship and sportsmanship are musts. Any parent, player, cheerleader or sibling who behaves inappropriately, uses vulgarity, or in any way causes a physical altercations, will be subject to board action up to and including expulsion from the program. (____) (____)
2. Parents should clearly understand that the goal of the Cordova Jr. Lancers is to teach their children teamwork, good sportsmanship, fair play, and a love for the sport of football and cheer. (____) (____)
3. Alcohol and/or drugs at practices or games are strictly prohibited. (____) (____)
4. Parents are never allowed on the practice field during practice. Parents are not allowed on the sidelines or playing field. All spectators must remain in the stands. Only players, cheerleader, coaches, and appointed association officials may be on the sidelines or on the track surrounding the field during the games. (____) (____)
5. Youth football is not professional football. Games are played under the current High School rules. Parents should cheer loud for their team, but your team support must be kept in the proper perspective and always in good taste. (____) (____)
6. Unsportsmanlike conduct in the stands during games is prohibited and may result in expulsion from the Cordova Jr. Lancers organization. Parents are responsible for the behavior of their invited guests – friends, family members etc. (____) (____)
7. Derogatory statements toward game officials or the opposing team are prohibited and may result in the forfeiture of a game and expulsion from the Cordova Jr. Lancers organization. (____) (____)
8. A positive mental attitude, free from parental pressures, is an important ingredient for the success of any children participating in the program. Parents should refrain from open criticisms, especially in the presence of other players/cheerleaders in the program, coaches, or any personnel connected with the organization. (____) (____)
9. Parents/Guardians should encourage attendance at all practice sessions. Absences impede a player/cheerleader’s progress and hurt the team overall. Missing practices is a prime cause for not playing in a game and can lead to suspension from the team for repeated occurrences. Missing practices should be limited to illness or serious injuries; player/cheerleaders with minor injuries, although not able to participate in full contact, can still benefit from observing practices. 10. All questions or problems should be directed to a Cordova Jr. Lancers representative. Coaches should never be interrupted during practice. Coaches should be contacted before or after practices for individual problems or questions. (____) (____)
11. Parents/Guardians are responsible for getting player/cheerleaders to and from all games and practices. (____) (____)
12. The Cordova Jr. Lancers organization cannot be held responsible for the health and welfare of unattended children at practices and games. Siblings of players/cheerleaders will not be supervised. (____) (____)
13. Parents/Guardians accept complete financial responsibility for their child’s participation in the Cordova Jr. Lancers football/cheer program. This responsibility includes mandatory participation in the fundraising program and volunteer hours, the payment of all fees, and the timely return of equipment, fully cleaned and in a condition of reasonable wear from use, free from excessive damage. Equipment that is lost or stolen will be paid for at current market replacement value. (____) (____)
14. Parents/Guardians should understand that their personal medical insurance will be the primary carrier should any injury result from their child’s participation. (____) (____)

In summary, as a Parent/Guardian for the Cordova Jr. Lancers, I/We certify that we have read and agree to abide by the Rules and Guidelines set forth in the Cordova Jr. Lancers Information and Registration Packets which were received at Registration. Most importantly the following:
(____) (____) I hereby agree to pay all required Registration fees, by the designated dates determined by the Cordova Jr. Lancers organization, and that my child will not be allowed to play football or cheer unless all fees are paid.

(____) (____) I hereby understand that the Cordova Jr. Lancers organization is run solely by volunteers, and I will commit at least 6 hours of volunteer time per child per family per season (12 hours max per family) to help make the program successful. I also understand that if I do not work my required hours, my $60 volunteer fee will not be returned in full and my child may not be able to participate in the program.
(____) (____) I herby agree to return football equipment (helmet, pads, pants, girdle, practice jersey) and cheer uniforms/ pom poms at the end of the season in good conditions. If Cordova Jr. Lancers equipment/uniforms are not returned in good condition, I will be charged the current market value of each missing item.
I have read the Parent / Guardian Responsibilities of the Cordova Jr. Lancers Football/Cheer Program and agree to all terms and conditions set forth.
