ASSESSMENT PROCEDURE INSTRUCTIONS FOR NEW STUDENTS

Hello,

 

I hope this document will walk you clearly through all the things needed to assess each new student and set up their folders for reading and math instruction.  Please read this whole document through before starting the assessment.  Once you understand the steps, use this sheet to help you go through the process with the child.  At first, this may take 30 or more minutes per child, but with experience, it will take less and less.

 

GATHERING MATERIALS

 

  1. First, go to the Tutoring Office located between the two 2nd floor classrooms.  Look on the Tutoring Bookshelf and remove the “Student Assessment Materials File Box”, the “Tutoring Supply Box”, and the “Tutoring Communication Notebook” from the bookshelf.  Look in the Tutoring File Cabinet’s lower drawer and remove a copy of the “Tutor Resource Materials Folder.”

 

  1. Take these materials into the Tutoring classroom.

 

  1. Remove a pencil for the child and a pen for yourself from the supply box.

 

  1. Next, set the “Student Assessment Materials File Box” on the table in front of you.  Look at the tabs sticking up.

 

  1. Remove the “Example Folder” from the “Example Folder” hanging file.  The child’s folders should mirror these examples at the end of the process.

 

  1. Remove one each of the prepared blank “New Reading Folders” and “New Math Folders” from the hanging files near the front of the “Student Assessment Materials File Box”. 

 

  1. Remove a copy of this document from the blank “Reading Folder” in your hand.  If the folder does not have a copy of this sheet, look in the “Assessment Instructions” hanging file.  Follow the steps outlined below to assess the child and to prepare his/her reading and math folders.

 

  1. Take a blank green hanging file with a blank name label from the back of the “Student Assessment Materials File Box”.  A name label should be in the folder.  If not, look in the “Tutoring Supply Box.”. 

 

  1. You are now ready to bring your child into the classroom for his/her assessment.  Label the green hanging file and both the manila reading folder and the manila math folder with the child’s name(last name, first name).   Please fill out these forms by yourself, do not have the child do so.

 

  1. Open the “Reading folder” and look at the left inner page called the “Reading Pupil Primary Assessment Sheet”.   Obtain and fill in the child’s personal information (name, age, school, etc) at the top half of the page along with your observations.   Please put the child’s name on the “Tutor Session Notes” sheet in both the math and reading folders.  Use the “Tutor Session Notes” for recording your thoughts and comments about how the child performs during later sessions.  You are now ready to use the SRA reading assessment materials to determine the child’s reading level.

 

ASSESSING THE CHILD

 

  1. Look in the “Student Assessment Materials File Box” for a copy of the Assessment Packet.  There are 4 copies available (A, B, C, D) so up to 4 children can be assessed at one time. The file box also contains copies of the assessment answer sheets for each exercise.  There are six types labeled “assessment answer sheets 1 - 6”. Remove a manila folder with one of the copies of the Assessment Packet.

 

  1. The SRA assessment packet has sections on phonics, on getting the main idea, on finding details, on comparing and contrasting, on cause and effect, on identifying fact and opinion, on sequencing, and on drawing conclusions.   The assessment packet tests proficiency in these areas.

 

  1. Please look at the first page of the packet, labeled “Phonics/Word study”.  Notice the red circle around the paragraph listing the type of assessment answer sheet that you will need for this section of tests.  The first page of each section has a similar paragraph directing you to the appropriate answer sheets.  The answer sheets are in the next hanging file to the rear, labeled “Assessment Answer Sheets 1-6.”

 

  1. You do not have to do an assessment in all the areas in the packet.  Phonics is the only mandatory area.  I would do phonics and one or two other areas of your choosing.  You should work through the pages with the child and see how they do in each case.  Put notes on the “Tutor Session Notes” sheet in the reading folder.  Hopefully, you will be able to assess the child and fill out the folders within a half hour.

 

  1. Using the placement chart (circled in green marker) on the front page of each test section, you can determine by the number missed on that section, the letter level the child should begin with.  Keep in mind that the chart may not accurately place a child if they get all questions correct, but have trouble with the many words or are very slow.  Better to start at a lower level and skip ahead in the material than to be frustrated from the beginning.  I generally place the child at the level before they begin to have trouble or go too slow.

 

  1. Write the letter level the child should begin at in phonics/word study on the lower part of the “Reading Pupil Primary Assessment Sheet” in the manila reading folder.  Test another one or two areas in the series and record the appropriate level as before.

 

  1. If the child cannot read, write a note to that effect in the tutor session notes.  Find out if they know the alphabet.  Indicate on the “Reading Pupil Primary Assessment Sheet” that they need to start with the picture level SRA materials and with the phonics exercise booklets on the tutoring bookshelf

 

  1. Return the child’s reading and math folders to the green hanging file and place the hanging file alphabetically by last name in the  “Student Folders” file drawer in the tutoring office. 

 

  1. Please put a note in the purple spiral “Tutoring Communication Notebook” with the child’s name and when they were assessed so we can add the child’s name to our lists.  Address the note in the communication book to the “tutoring program// date//new student” and circle it.  Also write in the child’s name on the record of each tutoring session clipboard below the bulletin board in the front room and record your session.  Remember to record your visit on the PREP tutor sign-in sheet.   

 

You have now completed the assessment of this new child.  Please return all materials to their appropriate hanging files, file drawers, and bookshelves in the tutoring office.  Thanks for your time and effort.
