Early swimmer registration has begun. If your child is planning to swim for the Thompson Park Dolphins again this year, please

• Read the Parent Newsletter

• Read and Sign Parent Participation Contract

• Fill in the Emergency Medical Form/Registration Form (front and back)

• Fill in the Product Order Form

• Make checks payable to Thompson Park Dolphin Swim Team

• Drop them off at Registration on April 18 between 10am and 3pm at the Calcutta YMCA or mail them to: Team Registrar

I UNDERSTAND THAT IT IS A REQUIREMENT THAT MY FAMILY PROVIDES AT LEAST ONE VOLUNTEER FOR EACH HOME SWIM MEET IN WHICH MY CHILD PARTICIPATES IN ANY OF THE FOLLOWING AREAS:

Timer Seeding Sell Heat Sheets &/or 50/50 Tickets Concession Stand Runner

Ribbons Meet Marshall Data Entry Announcer Meet Set-Up Meet Clean-up
