Performances 5 minutes maximum. Applying for talent audition act does not entitle one to perform – the talent committee can choose only a limited numbers of acts.

RELEASE AND WAIVER OF LIABILITY AGREEMENT
 

I, _________________________________(Participant), acknowledge that I have voluntarily applied to participate in the following activities at the Elks Lodge in Sonora, CA.

 

Tuolumne County Talent Show

I am voluntarily participating in the above-described activities with knowledge of what is involved, and agree to assume any and all risks of bodily injury, death or property damage. I also agree to allow the SISonora to use my name, image, story, and vocal talents, in any way the fair deems viable to the promotion and betterment of the SISonora with no regard for remuneration and I do so freely of my own accord.

 

I also agree if selected to perform, to attend the show as scheduled. I agree to do so in a manner befitting the wholesome image of a representative Tuolumne County and the SISonora.

As consideration for being permitted by the SISonora to participate in these activities and use their facilities, I hereby agree that I, my assignees, heirs, distributees, next of kin, spouse and legal representatives guardians, will not make a claim against, sue, or attach the property of the SISonora, The Elks Lodge or any of their affiliated for injury or damage resulting from the negligence or other acts, however caused, by any director, employee, agent, or contractor of the SISOnora, the Elks Lodge or any of their affiliated organizations in the activities above. I forever release the SISonora, as a result of my participation described and any of their affiliated organizations from any and all action, claims, or demands that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives have for injury or now have or may hereafter damage resulting from my participation in the activities described above.

 

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE SISonora, THE ELKS LODGE AND/OR THEIR AFFILIATED ORGANIZATIONS AND SIGN IT OF MY OWN FREE WILL.
